These Terms of Service (“Terms”) govern your access to and use of the Pacific Coastal Men’s Clinic website, patient portal, telehealth platform, and related services. By accessing this website or using Clinic services, you agree to be bound by these Terms.

1. Telehealth Services

Pacific Coastal Men’s Clinic provides telehealth medical services to eligible patients located within the State of Oregon.

Medical services are provided by licensed healthcare professionals acting within the scope of their professional licenses and in accordance with their independent medical judgment.

Use of this website does not establish a provider-patient relationship. A provider-patient relationship is established only after a licensed provider has accepted you as a patient and agreed to provide medical care.

2. Eligibility

To receive services, you must:

  • Be at least 18 years of age unless otherwise permitted by law.
  • Maintain an Oregon residence.
  • Be physically located within the State of Oregon at the time telehealth services are provided.
  • Provide accurate, complete, and truthful medical, identity, insurance, payment, and contact information.

The Clinic reserves the right to decline or discontinue services when clinically appropriate or otherwise permitted by applicable law.

3. No Guarantee of Treatment

Submitting an assessment, scheduling an appointment, enrolling in a membership, or paying for services does not guarantee acceptance as a patient, laboratory testing, prescriptions, specific medications, or any particular course of treatment.

All medical decisions are made solely by the treating provider based upon applicable medical standards, professional judgment, and federal and Oregon law.

4. Not Emergency Care

Pacific Coastal Men’s Clinic does not provide emergency medical services.

If you believe you are experiencing a medical emergency, call 911 immediately or proceed to the nearest emergency department.

5. Membership and Payment

Pacific Coastal Men’s Clinic operates as a membership-based medical practice.

Membership provides ongoing physician-directed medical management. When clinically appropriate, prescribed medications may be provided as one component of a patient’s treatment program.

Payment and refund practices are governed by the Clinic’s Membership, Payment, Cancellation, and Refund Policy, which is incorporated into these Terms by reference.

6. User Responsibilities

You agree to:

  • Provide accurate, complete, and current information.
  • Maintain the confidentiality of your login credentials.
  • Use Clinic systems only for lawful purposes.
  • Comply with all applicable laws.
  • Refrain from interfering with or attempting to compromise Clinic technology or operations.

7. Professional Conduct and Termination of Services

Pacific Coastal Men’s Clinic is committed to maintaining a safe, respectful, and professional environment for everyone involved, including but not limited to patients, prospective patients, family members, visitors, providers, employees, contractors, vendors, and other representatives of the Clinic.

Patients are expected to communicate respectfully during all interactions, including but not limited to telehealth visits, telephone calls, secure messaging, email, text messaging, written correspondence, and social media communications.

To the fullest extent permitted by applicable law, the Clinic reserves the right to refuse or discontinue non-emergency services, decline future appointments, restrict access to Clinic communication systems, or terminate an existing provider-patient relationship when appropriate, including but not limited to circumstances involving:

  • Verbal abuse, harassment, intimidation, threats, discrimination, abusive, profane, obscene, or otherwise inappropriate conduct.
  • Violent, threatening, disruptive, or intimidating behavior.
  • False, misleading, incomplete, or fraudulent information.
  • Doctor shopping, diversion, forged documentation, or attempts to obtain medications through deception.
  • Repeated missed appointments or repeated failure to participate in scheduled telehealth visits.
  • Repeated failure to comply with treatment recommendations when continued care is no longer clinically appropriate.
  • Failure to pay for services in accordance with Clinic policies.
  • Misuse of Clinic communication systems, including but not limited to excessive, repetitive, abusive, harassing, or inappropriate messaging.
  • Conduct that jeopardizes the safety, security, privacy, integrity, or operation of the Clinic.
  • Requests for treatment that are unsafe, unethical, outside the provider’s scope of practice, inconsistent with accepted standards of care, or prohibited by law.
  • Violation of these Terms, Clinic policies, or applicable law.
  • Any other circumstance in which continued care is no longer clinically appropriate or termination is otherwise permitted by law.

Nothing in these Terms obligates any provider to prescribe medication, order testing, continue treatment, or establish or maintain a provider-patient relationship when doing so would conflict with professional judgment, ethics, Clinic policy, or applicable law.

When termination of an established provider-patient relationship is appropriate, Pacific Coastal Men’s Clinic will make reasonable efforts to comply with applicable federal and Oregon law regarding continuity of care.

8. Messaging

Secure messaging is intended for routine, non-urgent communication only.

The Clinic may determine that a telephone call, telehealth visit, or in-person evaluation is medically necessary rather than responding through messaging.

Messaging should never be used for emergencies.

9. Intellectual Property

All website content, including but not limited to text, graphics, logos, images, educational materials, software, videos, downloads, and website design, is the property of Pacific Coastal Men’s Clinic or its licensors and is protected by applicable intellectual property laws.

10. Disclaimer

Information available through this website is provided for general educational purposes only and does not constitute individualized medical advice, diagnosis, or treatment.

11. Limitation of Liability

To the fullest extent permitted by law, Pacific Coastal Men’s Clinic shall not be liable for indirect, incidental, consequential, special, exemplary, or punitive damages arising from use of the website or Clinic services.

Nothing in these Terms limits any rights that cannot legally be waived.

12. Governing Law

These Terms are governed by the laws of the State of Oregon.

13. Changes

The Clinic may modify these Terms at any time. Updated versions become effective upon posting to this website.
